All clients are required to have updated forms for their accounts to continue services with us.

Clients booking BODY CONTOURING SERVICES  must complete BOTH forms.

Form A – Confidential Client Intake Form

Form B – Manual Lymphatic Drainage Consent
Form C – Post Op Manual Lymphatic Drainage Intake
 

Completed forms are REQUIRED prior to your appointment. You can complete and submit them easily on any desk top or mobile device. If they are not completed in advance, then they will be required and be part of your scheduled appointment time for services. NO SERVICES WILL BE COMPLETED WITHOUT THE PROPER FORM COMPLETIONS.
